The Dementia Partnership Project (DPP) is a partnership between Department of Health, Western Australia and Alzheimer’s WA aimed at improving the support provided to people living with dementia in the community. The project’s primary aim is to build capacity within the community care sector through a variety of ways that focus on enabling providers and health professionals to better respond to the needs of people living with dementia.
The project does not deliver direct care services but utilises a ‘train the trainer’ change management model to build capacity in organisations in order to improve service delivery with sustainable outcomes.
The focus is on the person’s journey from referral to service delivery by looking at a variety of activities and strategies to enable the community care sector to enhance the way in which they work with clients, each other and the sector.
The overall objective is to enhance the experience and improve the quality of life of people living with dementia in the community who access support services.
A model will be utilised initially by the DPP team to guide the capacity building, culture change process. However, it is hoped that feedback from pilot evaluations will assist in developing a robust framework that can be shared with other community care service providers and enable them to capacity build from within. A framework will incorporate the principles, processes and descriptions of the major activities and foundation tools and materials utilised by the team. It must be noted that the framework is not intended to be prescriptive. The needs of partner organisations during the analysis period will provide the DPP team and partner organisations with knowledge and understanding of the current state, gaps, enablers and drivers required to enable the development of a tailor-made work plan.
